Who We Are

As a patient in our clinic, it is your responsibility to keep scheduled appointments. The clinic requires that all appointment cancellations be received by 3:00pm on the prior business day (for example by 3:00pm on Friday for a Monday appointment).

Failure to cancel the appointment before 3:00pm the business day prior to the scheduled appointment will result in the assessment of a failed appointment fee of $100.

Any appointment scheduled and canceled on the same day will also result in a failed appointment fee.

You will always see the same provider.  

We recommend 15 minutes before your first appointment and 5 minutes for all follow-ups.

Call the crisis line or 911, or go to the nearest emergency department for urgent matters. Non-urgent matters are handled Monday through Thursday 9-5 pm, such as making and canceling appointments, medication refills, and billing questions.

Yes, family members can sit in the appointment with the permission of the patient. 

Yes. In Arizona, Nurse Practitioners may practice independently and do not require physician supervision or a collaborative agreement.

Your provider may fill out FMLA paperwork in an appointment only. Please notify the scheduler in advance that you have paperwork. Failure to mention your paperwork may result in the need to return for an additional appointment.

Strategic Mental Health does not fill out Long-term disability and Short-term disability.

The first appointment is an hour, depending on the severity of the case.
